Form Updates – DR 212 (Alternative Language Formats)

The DR 212 – Notice of Eligibility and Priority for Services has been updated and translated in the following alternative language formats:

  • Armenian
  • Spanish
  • Tagalog

These versions are available on the DOR Intranet, Forms Management page, using the forms search by Form Number (without DR prefix), Form Title or Category.
