The new revision to DR 210 – Enrollment for Vocational Rehabilitation Services has been translated in alternative languages formats to Armenian, Spanish, and Tagalog.  This revision aligns with the recent changes to the DR 222 – Vocational Rehabilitation Services Application and either the DR 210 or the DR 222 can be used for the application. 

The new form can be found on the DOR Intranet, Forms Management, using the forms search by Form Number (without DR prefix), Form Title or Category.
