A new form, DR 397R, has been established to document relevant information requested for the purposes mentioned in Workforce Innovation and Opportunity Act Section 511 34 CFR 397.

The intended use is for U.S. Dept. of Labor 14(c) Certificate Holders to proactively notify the DOR Achieving Community Employment (ACE) Team of individuals with disabilities who are newly hired for subminimum wage employment or placed in non-integrated work settings. 

The new form is available on the DOR Intranet, Forms Management using the forms search by Form Number (without DR prefix), Form Title or Category and/or in AWARE.
